Every three years, Dartmouth Health members and partners conduct a Community Health Needs Assessment (CHNA) – a 10 minute survey – where we ask community members to tell us about their health needs and priorities.
During our last cycle, over 1800 members of the DHMC and APD service region took our survey. Based on the results, we continued investing in substance misuse prevention, housing, and access to healthcare.

Every Wednesday, CommunityCare of Lyme lifts up a wellness topic, embracing the widest possible definition of individual and community well-being. We include local and national resources, individual and group programs and practices, and personal stories, videos, or songs that have something to teach us all. We are always seeking guest contributors!
